PRIVACY POLICY:  This health service business, The Lifetime Body, PLLC, in compliance with the Health Information Portability and Accountability Act, maintains strict standards concerning the confidentiality of all personal, financial, and medical records. Access to such information within this organization is limited to authorized personnel for official purposes only. Disclosure of any of this information is only done with your permission and is limited to the factual information necessary for personal health and medical records.  You may also disallow disclosure of these records to others. We request that you provide us with your email address to enhance convenience for communication regarding scheduling and billing. Under no circumstances will this email address be given to anyone for any reason. We provide this information to you not only because we wish to assure you of your legal rights and privacies, but because it is our goal that you are satisfied with our services on all levels.

SERVICE POLICY:  The Lifetime Body (TLB) makes every effort to provide all clients with the most up-to-date and effective information and service available for optimal health and fitness promotion. In accordance with this service policy, you consent to receive services and education as provided by TLB. Additionally, you recognize that you are ultimately solely responsible for your health, and waive all responsibility from TLB for any outcome that results from the receipt of any service provided by TLB.

PAYMENT POLICY:  Payment for all services is due at the time of scheduling. This includes physical therapy sessions, consultative appointments, coaching relationships, and presentations/educational programs. For physical therapy services, all clients will be issued documentation that can be submitted to their insurance carrier for processing according to their specific policy. TLB cannot provide physical therapy services (this exception does not apply to fitness, wellness education, and coaching services) to Medicare beneficiaries.

SCHEDULING POLICY: All appointments must be scheduled a minimum of 12 hours prior to the time of the session. All sessions are non-refundable but have unlimited reschedule options.
